HOW TO SUBMIT YOUR APPLICATION
Applying for your Preliminary Assessment is
easy - simply read the following information, then follow the
instructions provided on each page.
Forms
In order to undertake your
Preliminary Assessment we need your completed Application Form. Please
ensure that you have included all the required information, have signed
and dated the form and indicated which payment method you have chosen.
Your completed form should then be posted to our National Service
Promotion Associate, ITL, ITL House, School Lane, Leyland, Lancashire
PR25 2TU
Payment Options
There are two payment options
available to you.
Option 1 - Credit/Debit Card Payments
Credit/Debit Card payments are available by telephone during office
hours (9am-5pm Mon-Fri) by calling 01772 621909
If you choose to pay using your credit or debit card, you will be
given a unique transaction number which should be included on your
Application Form.
Option 2 - Cheque Payments
Your may also pay for your Preliminary Assessment by cheque.
Please make you cheque payable to Land Planning Associates and enclose this with your
completed Application Form.
Completing your
Application
There are three easy steps to complete your application -
Step 1 - Click the STEP 1 link at the foot of this page -
Your will be taken to our Statutory Information page. Please note you
are required to read this by law.
Step 2 - Once you have read the Statutory Information section, click
the STEP 2 link at the foot of the page to go to the Application
Form.
Step 3 - Print this page! - If you have elected to pay by cheque, complete the
Application Form, enclose your payment and post.
If you have chosen to pay using your Debit/Credit card - call 01772
621909 to make your payment - enter your transaction number
on your Application Form and post.
